Please plan to arrive 15 minutes early for your first visit to Davison Dental in order to complete the brief medical history forms, or you may print them from this website and bring completed forms to your first appointment.
If you have recent dental x-rays (obtained within the last 12 months), please bring them to your initial appointment. If you are being referred to Davison Dental from another dentist, specialist, or patient, please let us know who referred you to our practice. We like to thank those that recognize the high quality of our care.
What do I need for my first visit?
We will need your driver's license, insurance card, and a list of any medications you are taking, including over-the-counter medications. If you have been instructed in the past to take antibiotics before any dental procedure, please call us to verify if it will be necessary for your initial visit.
